Rev. Jerry Ray Cook, age 72 of Collinsville (Lebanon Community), passed away Wednesday, August 9, 2023. Funeral Services will be held at 2pm on Friday, August 11, 2023 from the graveside of Lebanon Cemetery with burial to follow. Officiating is Bro. Herman Thompson.
